Insurance Programs offered through ABWA Include:

12 month Short Term Medical Insurance
A temporary medical insurance for students, graduates, self-employed or laid off people or anyone without medical coverage for any reason. Also good if you have a waiting period until your medical insurance is in effect. Runs for 12 months BUT it does have medical questions that must be completed. 

First you pay the deductible: $250 or $500 of covered expenses. Plan pays 80% of the next $5000 of covered expenses. The Plan then pays 100% of the remaining covered expenses up to a Lifetime Maximum of $1,000,000

Often used by students that have been dropped from parent’s plan due to age. 

Includes a prescription drug DISCOUNT card. 35,000 pharmacies that participate in the program and offer members discounts up to 65% of retail cost. Discounts vary upon prescription.

Note: You have 5 medical questions to answer on the application. All must be answered “NO”. Any yes answer, YOU DO NOT QUALIFY.


Cancer Plan 

This plan is similar to the stand alone policies offered by AFLAC
Underwritten by Monumental Life Insurance Company
Member and family price $15.07/month
Member only $ 9.42/month

Pays in addition to any other coverage -- Checks payable directly to you.

No Blood Tests, No Medical Exam, No AGENT, 
If you haven’t had cancer in the past 10 years, you’re accepted. BUY IT WHILE YOU ARE CANCER FREE.

Has NO deductibles or copays. It pays cash benefits immediately and IN ADDITION to any other health plans you have.
Benefits include: Up to $250 cash a day for hospitalization (1-60 DAYS) Extended hospital up to $250 after 61 consecutive days in hospital
Up to $100 cash a day for intensive care (ICU for cancer – maximum benefit 15 days)
Up to $100.00 cash a day for outpatient treatment including radiation & chemotherapy
Plus $1,000.00 for the first diagnosis of cancer. 

Must send quarterly check with application. 

Disability Income Insurance  
You insure your home, car, life – why not insure your income!!!!

Note: Many people think they will have coverage at work such as thru workmen’s comp which only covers for injuries you have as a result of a work injury. This is an area where many find themselves in big trouble -- Illness, car accident, etc. do not fall under workmen’s comp. Check with your place of employment to see if you have disability income insurance thru your employer. Many DO NOT.

Underwritten by Hartford Insurance.  Two types of plans:

Plan 1 – pays monthly benefit for up to one full year for a covered total disability caused by accident or illness.
A spouse can also apply for coverage only under Plan 1.
Note:   Elimination Period: Benefit only begins on the 31st day for up to one full year if totally disabled by a covered accident or illness.

Plan II – Pays monthly benefit as long as you are totally disabled up to age 65. If you become totally disabled after 65, you are paid every month for two full years. 
Note: Elimination Period: begins the 31st day of total disability.

You select the monthly benefit you want -- Select from $500 to $5000 a month. You are elegible to apply for up to 66% of your gross monthly salary when combined with Social Security, Worker’s compensation, or any group or other disability policy

Pay NO premiums while disabled.

Pre-Existing Conditions – this is when a person received medical treatment, care or advice within 12 months before being covered by this policy. Under Plan I only, disabilities caused by pre-existing conditions will not be covered until the insured has received no such treatment, care or advice for 12 consectutive months or until the covered person’s insurance has been in effect for 24 consecutive months, whichever comes first. This limitation DOES NOT apply to members covered under Plan II 

(NOTE: WILL NOT PAY FOR PREGNANCY, CHILDBIRTH, ABORTION, MISCARRIAGE

Send NO money with application because they calcuate each premium based on your age and amount of monthly benefit selected. They will bill you.


Catastrophe Major Medical Insurance Plan
You must have a primary medical insurance plan. This is to supplement your plan.

A deductible of $25,000 – this means after your primary insurance company has paid bills amounting to $25,000, then the deductible for your catastrophe major medical insurance plan has been met. 

Take up to 3 full years to satisfy your deductible.
Pays hospital charges or intensive care. Supplies and services used while in hospital. Charges by licensed physician for diagnosis, treatment and surgery. 

Private duty nursing service while in hospital or at home -- $120 maximum per 8 hour shift up to a lifetime maximum of $35,000 per insured.

Ambulance service up to $2000 lifetime max per insured.
Anesthetic and its administration.
Prescription drugs, casts, splints, braces, crutches both in and out of hospital. 
Oxygen and rental of equipment such as hospital beds, wheelchairs, etc.
Mental or nervous disorders, alcoholism or drug addition treated in a hospital up to $25,000 lifetime maximum
Up to 100 visits per calendar year for Home Health Care Treatment.
Plus Reasonable and customary expenses for custodial care as an inpatient in a nursing/convalescent home – up to $500 per week for up to 3 years (lifetime maximum) following an elimination period of 6 days.
Note:   Currently nursing homes in our area range anywhere from $300 to $500 PER DAY
